In certain cases multiple doctors from different clinics may create a patient's file, resulting in multiple profiles. To merge patient profiles across clinics please follow the steps below:
- Request access to any patient profiles that you do not already have access to by following the instructions found here. It is important to note that you must request access to the entire patient profile (rather than just a select spot). If the patient has an active account, they will receive the email directly. For inactive accounts for which there is no email address on the patient profile, you will need to ask the doctor to share the patient file with you.
- Once you have access to all patient accounts, you may follow the steps found here to merge them into a single profile.
*Please be sure that you wish to merge the patients, as this cannot be undone.
