The Patient Consent Forms Dashboard is where you can add, enable, and edit consent forms for your patients to sign. Additionally, this is where you can create, edit and delete questionnaire forms to be included with your consent forms. To learn how to access your Patient Consent Forms Dashboard follow the instructions below:
- Select your profile from the top menu bar and click the Enterprise button from the drop-down menu.
- Click your desired clinic.
- Select Consent Forms from the left sidebar. Here, you have the option of previewing your forms in a PDF format, adding/editing/removing a Questionnaire, enabling/disabling a form, or editing a form respectively. To learn how to create a consent form click here.
