How do I add 'File Upload' question type to my forms?

In the custom form builder, you may want to ask a question that allows the user to upload a file type as the answer. To learn more about the custom form builder, click here. This article will teach you how to build a form with a 'file upload' question type.

  1. In the top right corner, select 'Manage Clinic' under your account and see 'Form Builder' on the left-hand side.


  2. Create your new form. Fill out the form name and section headings, then add the questions you want to ask. To add a question with a 'file upload' answer, select the 'Radio Buttons' to change it, and then select 'File Upload'. 

    • You can upload PNG, JPEG, PDF, and HEIC file types as an answer to this question. 
    • Each file must not exceed 50 MB, with a maximum of 10 files allowed per answer.

      *Note: dependent questions cannot be made based on the responses to 'file upload' questions.

      *Note: This feature is supported for patient and provider questionnaires on Android and iOS mobile apps; however, it is not available for intake and consent forms.

3. Click 'Save' to finish creating your form. 

4. You can now use your new form with the sections and questions that you have set.
