How Can I Fill Out a Patient Intake Form?

You have the ability to create a customized Patient Intake Form where you can fill out upon patient registration. To learn how to create your custom patient intake form, click here

  1. Select the Patients List option in the top menu bar.
  2. Select the Add Patient button.
  3. Here, you have the option of creating a new patient profile based off their First & Last Name or their Health ID. Note that in each scenario certain fields are required to be filled out to proceed:

    First & Last Name: First Name, Last Name & Sex.
    Health ID: Health ID & Sex.

    If you enter their email address, you will be prompted to select whether you would like to invite this patient to have the ability to share and access their images.

  4. You will be prompted to complete the Patient Intake Form which can then be edited at any time. 
  5. When ready, click Next.


iOS

You have the ability to create a customized Patient Intake Form where you can fill out upon patient registration. To learn how to create your custom patient intake form, click here . To get started, follow the steps below:

  1. Tap the mceclip0.png icon in the Patients List. 
  2. Enter the patient's First Name, Last Name and Sex. Optionally, you are also able to enter their Date of Birth, Ethnicity, Skin Type, Health History, Address and Phone Number (optional). When ready, tap the Register Patient button.
  3. You will be prompted to complete the custom patient intake form and select Next. 


Android

You have the ability to create a customized Patient Intake Form where you can fill out upon patient registration. To learn how to create your custom patient intake form, click here . To get started, follow the steps below:

  1. Tap the mceclip1.png icon in the Patients List. You are able to create a patient based on their full name or Health ID.
  2. Select the Full Name tab, then enter the patient's First Name, Last Name, and Sex(required). If you enter the email, you will be prompted to select if you would like to invite this patient to share and access emails (the patient will receive an email invitation).
  3. You will be prompted to complete the custom patient intake form and select Next.
