If you would like to have patients answer a few questions about the case with their submission, you can set them here. This feature enables you to easily create a list of text or multiple-choice questions, choose between mandatory or optional, and order them as needed.
- Log into your DermEngine account. Click on your name in the top right-hand corner and select Manage Clinic from the drop-down menu.
- Under 'Teledermatology' on the left sidebar, scroll down and select the 'Questionnaires' option.
- Select the skin conditions that your services will be providing to. You can also add custom conditions by clicking '+Add More'.
- You can edit or delete custom skin conditions at any time by clicking on and selecting 'edit' or 'delete'.

- Select whether you would like to edit the patient questionnaire or the questionnaire for medical professionals by clicking on the respective .
- Your form comprises of three main categories: Name, Sections, and Questions. The Form Name will be automatically labeled as Patient Questionnaire or Physician Questionnaire, and can be edited at any time. You will see a blank section labeled "Section 1". You have the option of changing the names of a Section or Question as desired.
- To edit the question type, select the arrow in the top right corner of the Question and then choose the option accordingly.
- If you have selected question types such as Radio Buttons, Checkboxes, or Dropdown, you can add your answer options by clicking the 'Add Option' button. You can also select the Slider option and enter a minimum and maximum value. If choosing the File Upload option, up to 10 files can be uploaded with the formatted jpg/jpeg, png, or pdf.
- Additionally, you can include an 'Other' option by clicking the 'Add Other' button.
- Once you have completed that question, you have the option of making the question 'Required' by selecting the toggle. You can also create a copy of the question by clicking .
- Repeat steps 5 through 7 for the necessary sections and questions. When ready, click Save.
- Once you've clicked 'Save', your questionnaire will be added the next time your patient submits a case in a teleconsultation.
